Requests should be made in writing and include:
- A cover sheet that includes your organization’s name, contact person, title, address, phone number and federal tax ID number
- A one-page cover letter outlining the proposal, including the project/program name and the total amount being requested
- Proposed use of the funds
- Total amount requested
- A copy of your organization’s letter from the IRS indicating its charitable 501(c)(3) tax-exempt status

Timeline for Requests
The MEDRAD Corporate Giving Committee meets quarterly to review proposals and requests. Our business is run on a calendar year;
January to March
April to June
July to September
October to December
Each applicant receives written notification that their application is received, including notification of the next meeting date. All applicants are notified in writing of the committee’s decision.
