A: A copy of a Field Service Report (FSR) is distributed to you after your MEDRAD Customer Service Representative (CSR) performs on-site service at your facility. Additionally, for billable service calls, a copy of the FSR is enclosed with your invoice. If additional copies are required, you may contact the MEDRAD Customer Support team at 1-800-633-7237 for assistance.
